REQUEST FOR SERVICE

After completing this form, you may fax the application to our office at 205-620-6560 then call our office at 205-620-6420 and pay your new service fee and/or deposit by mastercard or visa.  You must contact our office by phone after faxing your application or service will not be established.  You may mail the completed application along with a check for the applicable fees and/or deposit to:  Pelham Water Works, Post Office Box 1479, Pelham, AL  35124.  We must have the application and fees at least one business day before service can be established.

AUTOMATIC BANK DRAFT

After completing this form, you may fax it to our office at 205-620-6560 along with a copy of a voided check from the account you wish to have drafted or you may mail to:  Pelham Water Works, Post Office Box 1479, Pelham, AL  35124.  Please contact our office for the date your account will be drafted.

After completing either form, you may come to the Water Works Office to pay your new service fees and/or give us a voided check to set up your automatic bank draft.
